Job description
Beaumont Home Health in Michigan is seeking a Registered Nurse Case Manager (RNCM) to lead assessment, planning and coordination of care for patients in their homes. The RNCM provides direct clinical care, case management and supervision of care across disciplines under physician direction.
The RN educates patients and families, ensures safety, and maintains compliant documentation. A sign-on bonus of $10,000 is available; schedule is Monday–Friday, 8:30 am–5:00 pm.
