Job description
Alternate Solutions Health Network, LLC seeks a Registered Nurse Case Manager to lead assessment and care planning, coordinating disciplines and providing clinical care under physician direction. The RN educates patients and families and ensures safe, billable care with accurate documentation.
This role requires state RN licensure, strong communication, and the ability to manage complex cases while complying with regulations. On-call responsibilities and travel to patient locations are required.
