21st Century Home Health Services (21HHS) seeks an experienced Registered Nurse Case Manager, Level 2, to oversee patient care in Woodland and surrounding areas. You will lead two nurses, coordinate start of care, evaluations, and discharge planning, focusing on preventing re-hospitalization and promoting wellness in home settings.
Requirements include a CA RN license, BLS/CPR, and 1+ year of home health case management or nursing experience.
