Job description
21st Century Home Health Services (21HHS) seeks an experienced Registered Nurse Case Manager, Level 2, to lead patient care in Sonoma and surrounding areas. You will develop and execute plans of care to promote wellness and prevent readmission.
In this leadership role you’ll supervise two nurses, coordinate with physicians and an interdisciplinary team, and ensure timely charting and compliance. CA RN license and BLS/CPR are required.
