Job description
21st Century Home Health Services is seeking a Registered Nurse Case Manager, Level 2, to lead a small team in Sonoma and surrounding areas. You will assess patients in their homes, develop and implement an advanced care plan, and collaborate with physicians and an interdisciplinary team.
You will supervise two nurses, coordinate SOCs and discharges, and educate families on care, medications, and wound care.
