Job description
Henry Mayo Newhall Hospital in Santa Clarita seeks an RN Case Manager to coordinate care across the patient’s continuum, supporting discharge planning and post-hospital needs. You will interact with insurers and medical groups to ensure efficient use of services and clinical resources.
