Join our growing team! CareOne Home Health is locally owned and growing company seeking an experienced RN Case Manager to cover Monterey County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support. This is a per diem position with potential for full time as we grow.BenefitsCompetitive salaryFocused one to one patient careMedical, dental and vision insuranceFlexible work schedule & reasonable work hours401k matchingSupportive and friendly work environmentRequirementsMinimum of 1 year of experience as a Registered Nurse with a valid license requiredCurrent CPR/BLS CertificationReliable transportation requiredExperience with Kinnser strongly preferredComfortable driving to patient homes for patient visitsPrior experience with Palliative Care preferredPrior experience in Home Health, Hospice, Infusion, or Home Care preferredResponsibilitiesInitiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate.Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility.Communicate necessary revisions to patient status and/or needs with the entire care teamRegularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.Provide complete, accurate and timely documentation consistent with the plan of care
