nRN Case Manager
nPosition Overview
nThe Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient’s hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.
nEssential Functions and Skills Patient Care
nnnProvides care in accordance with the Nurse Practice Act and professional nursing standards.
n
nnConducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.
n
nnDevelops, implements, and revises the Plan of Care based on ongoing patient evaluations.
n
nnAdministers medications and treatments as prescribed in the physician’s plan of care.
n
nnEducates patients and families on self-care, medications, and diet as appropriate.
n
nnMaintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.
n
nnCoordinates care with physicians, interdisciplinary teams, and community health partners.
n
nnEnsures a safe environment for patients and supports their activities of daily living.
n
nnProvides emotional support and counseling to patients and families.
n
nnArrange for necessary medical equipment and supplies.
n
nnEnsures HIPAA compliance and patient confidentiality.
n
n nAdditional Duties
nnnParticipates in on-call visits as required including evenings and weekends.
n
nnAttend interdisciplinary group meetings.
n
nnSupervises ancillary personnel and delegates responsibilities as needed.
n
nnEngage in professional development through training and continuing education.
n
nnContributes to quality assessment and performance improvement activities.
n
nnPerforms other duties as assigned.

n nQualifications
nRequirements/Qualifications
nnnMust have a current Registered Nurse (RN) license in state(s) of practice.
n
nnNurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
n
nnGraduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
n
nnAbility to pass DHS background, consumer report, and drug test.
n
nnValid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
n
n nPreferred
nnn1+ years of recent experience in hospice/home care preferred.
n
nn2+ years of Nursing experience, preferred
n

nnAbility to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.

n nThe salary range for this role is $80,000-$96,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
