Job Description
Performs initial and ongoing clinical assessments via telephone calls to clients for the purpose of assessing client's situation with regards to healthcare needs, cultural implications and support systems in place.
Creates a case management plan based on the assessment with measurable goals and objectives utilizing evidence-based criteria; monitors ongoing progress toward these goals and objectives; implements plan through case management interventions and communicates with all parties to reach desired goals and objectives.
Evaluates need for alternative treatment.
Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex clinical indicators which impact care planning and resolution of patient issues.
Qualifications
Must have at least 3-5 years case management experience
Must have 1-2 years managed care experience
Must have Bachelors Degree in Nursing
Must have valid Florida RN License
Must be proficient with EMR
