Job description
Mercy is seeking an RN Case Manager to coordinate patient care across Watonga, Kingfisher, and Guthrie, ensuring safe transitions through the continuum. You will complete assessments, develop care plans, and collaborate with physicians, therapists, social workers, and community partners to optimize outcomes and reduce readmissions.
In Mercy’s mission-driven culture, you’ll educate patients and families, arrange resources, perform utilization review, and document in the EMR.
