Job description

HCA Florida North Florida Hospital seeks an RN Case Manager to guide patients and families through the care journey, coordinating services and developing personalized care plans. You will work with physicians, nurses, and leaders to ensure safe transitions and clear communication across teams.

Requirements include Florida RN license, BLS within 30 days, and 3 years RN acute care experience; BSN preferred and Case Management certification preferred.
