Job description
Carolina Family Health Centers, Inc. is seeking a Case Manager II, Registered Nurse - Population Health Medicaid to provide medical case management for a defined patient panel in a clinical setting.
The RN will assess needs, develop person-centered care plans, and coordinate referrals with community resources while ensuring quality care and patient engagement. This is a full-time, on-site role offering comprehensive benefits and travel as needed.
