Job Title
Home Health RN Case Manager (RNCM) + Recertification Nurse (PRN or Contractor)
Job Summary
As an RN Case Manager, you will coordinate and provide patient care across Indiana. The role combines RNCM services, recertification visits, and occasional State Survey support from our Indianapolis office. The position is PRN‑only with potential to grow into part‑time or full‑time as the agency expands.
Employment Type & Schedule
PRN (as needed) or Contractor.
Typically 1–15 daytime hours per week, Monday–Friday 8:00 AM–5:00 PM.
Occasional early‑evening or weekend visits are rarely required.
No overnight or overtime shifts.
Visits scheduled as close to your home as possible.
Hourly compensation $45–$55 per hour, paid weekly via direct deposit, plus door‑to‑door mileage reimbursement.
Service Area
Indianapolis, IN and surrounding 1‑hour radius (up to 2‑hour radius coverage optionally). Travel generally 10–60 minutes; occasional 2‑hour travel may be required.
Job Location Options
Primarily serves Marion County, but applicants residing nearby are considered if travel requirements are met.
Available office in Indianapolis at 8580 Cedar Place Dr, Indianapolis, IN 46240.
Responsibilities
Conduct client assessments, care planning, and coordination of services.
Perform admissions, recertifications, transition care, and resumptions of care.
Provide clinical reviews, OASIS/non‑OASIS assessments, wound care, PICC/PORT/IV care, and lab draws.
Supervise and support RN, LPN, Home Health Aide, and Personal Care Aide staff.
Document and communicate patient status changes with the care team.
Support State Survey visits (1–2 times per year) from the Indianapolis office.
Maintain accurate documentation and field‑staff leadership.
Travel to client locations across Indiana, including the service area.
Qualifications
Graduate of an accredited School of Nursing.
Active RN license in Indiana.
Minimum 1 year of home‑health or in‑home care experience; 2–5 years preferred.
Minimum 1 year of case‑management experience.
Experience with OASIS assessments, wound care, lab draws, and IV care.
CPR and First Aid certifications (current or obtainable before hire).
Strong documentation, field‑staff leadership, and supervisory skills.
Reliable transportation and valid driver’s license.
Ability to work flexible days and travel within the service area.
Desirable but Not Required
Nurse supervision experience.
People management and staff education.
Relationships with client payors and case‑management entities.
