Job description
Madonna Rehabilitation Hospitals is seeking an RN Case Manager to lead interdisciplinary care planning within the Case Management department. The role emphasizes coordinating patient plans, liaising with families, payers and physicians, and ensuring quality, cost-effective outcomes in a rehabilitation setting.
The position requires Nebraska RN licensure, 18+ months of clinical experience (1 year in rehab preferred) and knowledge of Medicare/Medicaid programs. CCM is preferred.
