Job description
Sanford Health in Marshfield, WI is seeking a part-time RN case manager to oversee patient care coordination on the 5W Inpatient Rehab unit. You will collaborate with licensed practitioners and the inter-professional team to ensure effective discharge planning and clinical outcomes.
Qualified candidates hold a nursing degree with an unencumbered RN license and BLS within 60 days of hire; CCM certification is encouraged.
