Are you a compassionate Registered Nurse who thrives on building relationships and
making sure patients receive the right care, at the right time, in the comfort of their own
homes? We’re looking for a Home Health Case Manager who’s organized, clinically skilled,

What You'll Be Doing
● Manage the Care Journey: Serve as the primary point of contact for a caseload of home
health patients, ensuring seamless coordination of services.
● Conduct Comprehensive Assessments: Complete OASIS assessments and develop
individualized care plans in collaboration with the interdisciplinary team.
● Coordinate Services: Work with physicians, nurses, therapists, social workers, and home
health aides to ensure timely delivery of care and optimal outcomes.
● Monitor and Evaluate: Track patient progress, update care plans, and communicate with
families and caregivers as needed.
● Documentation: Maintain accurate and timely clinical documentation in accordance
with Medicare guidelines and agency policy.
● Advocate for the Patient: Provide education, resources, and emotional support to
patients and their families to promote independence and wellness.
What We’re Looking For
● License: RN (Registered Nurse) with active(state) license.
● Experience: At least 1 year of experience in home health or case management preferred.
● Skills: Strong critical thinking, communication, time management, and the ability to
work independently.
● Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB,
or WellSky is a plus).
● Reliable Transportation: Valid driver’s license, auto insurance, and access to a
dependable vehicle.
