Job Summary
Health Services is seeking an RN Case Manager to coordinate and monitor student care across campus, community, and private healthcare services. Reporting to the Director of Health Services, this position supports students in navigating referrals, insurance resources, mental and physical healthcare services, and follow-up care. The RN Case Manager also provides professional nursing services in an outpatient student health setting and collaborates with healthcare providers, campus departments, and community agencies to promote continuity of care and student success.
Key Responsibilities
Coordinate care and follow-up for students receiving multiple Student Wellness Center services, including mental health, eating disorder, sexual health, and gender-affirming care
Assess student needs, triage referrals, and connect students with appropriate campus, community, private, and insurance resources
Support healthcare providers with referrals, aftercare planning, medical records, prescription refill requests, and continuity of care for high-risk students
Provide nursing services, including triage, vital signs, immunizations, medication administration, testing, treatment assistance, and electronic health record documentation
Develop referral networks, participate in quality improvement and accreditation activities, and support Student Wellness Center programs and campus events
Knowledge, Skills & Abilities
Ability to effectively establish and maintain cooperative working relationships within a diverse multicultural environment
Ability to maintain confidentiality and knowledge of confidentiality requirements within healthcare and/or education settings (e.g. HIPAA, FERPA, etc.)
General awareness of emerging technologies, including artificial intelligence (AI), and a willingness to develop skills in their appropriate, responsible, and ethical use in a higher education environment, consistent with institutional policies and practices
Knowledge of local/state community resources, including ability to navigate a higher education campus and the resources offered therein
Thorough knowledge of general methods, procedures, and practices of an outpatient medical facility
Ability to effectively apply nursing knowledge and demonstrate proper techniques and skills in performing nursing duties
Effective communication across a variety of venues. Working knowledge of English grammar, punctuation, and spelling.
Working knowledge of electronic health record systems and ability to learn new systems and assess appropriate workflows within EHRs to manage patient cases
Ability to handle multiple work priorities, organize, and plan work and projects
Skill in motivational interviewing techniques and ability to conduct individual meetings. Must possess excellent customer service and public relations skills
Thorough knowledge of professional nursing principles, methods and procedures; anatomy and physiology; medical supplies and equipment used in nursing practice; and the uses and effects of medications and narcotics
Ability to analyze emergency situations and respond promptly and stay calm
Ability to take initiative and propose new processes and/or process improvements.
Ability to problem solve and address confusion and frustration with multi-step processes
Required Qualifications
High School diploma or equivalent
Completion of a professional nursing program in an approved school of nursing or by additional preparation as approved by the California Board of Registered Nursing
Possess and maintain a valid license as a registered nurse in the State of California and certification in CPR/AED
One year of progressively responsible full-time professional nursing experience or equivalent combination of education and experience which provides the required knowledge and abilities
Preferred Qualifications
At least one (1) year of experience providing case management in a nursing capacity.
Demonstrated experience assisting patients navigating referrals and/or insurance resources.
Demonstrated experience working within an integrated health facility
Compensation
Classification: Registered Nurse II
Anticipated Hiring Range: $8,588/month - $8,935/month (Step 16 – Step 18)
CSU Salary Range: $6,381/month - $9,296/month (Step 1 – Step 20)
