Job description
Massachusetts General Hospital is seeking a Registered Nurse to join the Case Management team in a 32-hour-per-week, onsite role at 55 Fruit Street. The RN Case Manager collaborates to optimize patient care across the continuum, coordinating assessment, planning, implementation, care coordination, and discharge planning to improve outcomes.
Requirements include MA RN license and at least 2 years Med/Surg experience; Case Manager certification is preferred. ADN required, BSN strongly preferred.
