Job description
Whittier Rehabilitation Hospital is seeking a seasoned Case Manager/Discharge Planner (RN preferred) for our 20-bed Transitional Care Unit. This full-time role coordinates admission to discharge, collaborates with the care team, and guides patients and families through the rehabilitation process in a CMS-guided environment.
Requirements include an RN license with 2–5 years acute case management in SNF, strong communication skills, and knowledge of CMS guidelines. Sign-on bonus available.
