Job description
Rocky Mountain Care Group in Vernal, UT seeks an experienced RN Case Manager for our home health and hospice team. You will coordinate patient care, perform visits, and liaise with physicians and office staff to ensure timely, quality care.
The role requires an active RN or LPN license, CPR certification, and strong communication and critical thinking skills to manage complex cases in a fast-paced environment.
