Job description
John Muir Health is seeking an RN Case Manager to coordinate and advocate for patient care from hospitalization through transition to the community. The role emphasizes care coordination, utilization management, and patient education within the full scope of nursing practice.
The position requires a BSN, 3 years of nursing experience (medical/surgical preferred or critical care), and active RN licensure. Proficiency with EPIC and other care-management tools is expected.
