Hospice Registered Nurse (RN) Case Manager – Full-Time | Vernon Hills, IL

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area.
Schedule & Territory
Territory: Vernon Hills, 60061 and surrounding communities.
Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
$80,000 – $97,000 annually (based on experience)

Key Responsibilities
Manage a dedicated hospice caseload as the primary RN Case Manager
Perform patient assessments, admissions, and individualized care planning
Deliver skilled nursing care focused on comfort and dignity
Coordinate care with physicians and interdisciplinary team members
Conduct routine visits and adjust care plans as neededProvide end-of-life support, including family education and pronouncement
Supervise Home Health Aides in compliance with regulations
Qualifications
Graduate of an accredited RN program
Current CPR and First Aid certification
Hospice or home health experience preferred
Strong clinical judgment and communication skills
Valid driver’s license and proof of insurance
