Job description
Decypher seeks an RN Case Manager to deliver comprehensive case management through assessment, care planning, coordination, implementation, monitoring, and evaluation to promote continuity of care and optimal health outcomes.
Requires ADN (or BSN), active RN license, and several certifications (CCM, BLS, COHN/COHN-S, CRC, ACCC among others). U.S. Citizenship is required and prior discharge planning experience is expected.
