nWe are seeking an experienced and compassionate Hospice Clinical Liaison to join our team. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.

nEssential Responsibilities

nnnReceive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals

n

nnAssess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families

n

nnEducate patients and families on what hospice care includes, how it works, and why it matters—providing clarity, reassurance, and compassion

n

nnComplete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met

n

nnServe as a liaison between referral sources and our hospice care team—ensuring seamless handoffs, clear communication, and rapid response to needs

n

nnMake daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians

n

nnBuild and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services

n

n nAbout You

nEducation

nnnLicensed Nurse with three to five years of hospice experience required.

n

nnBSN with five or more years of hospice experience preferred.

n

n nExperience

nnnMinimum of three years of hospice experience required.

n

nnPrior experience as a discharge planner, case manager, or educator preferred.

n

n nLicenses and Certifications

nnnCurrent RN license to practice nursing in the state of employment.

n

nnCPR certification.

n

nnValid driver’s license and automobile insurance.

n

n nSkills and Knowledge

nnnStrong knowledge of hospice care and the unique needs of patients and families.

n

nnFamiliarity with community resources and referral networks.

n

nnExcellent patient assessment and documentation skills.

n

nnAbility to supervise staff, coordinate services, and manage programs effectively.

n

nnStrong communication and interpersonal skills with sensitivity to diverse backgrounds.

n

nnnCompassion and sensitivity to the needs of terminally ill patients and their families.

nnFlexibility to manage a variety of tasks and shifting priorities.
