RN Clinical Manager – Home Health Services

We’re seeking aRegistered Nurse (RN) – Clinical Manager to oversee clinical operations and patient care services in our home health division. This is your opportunity to lead with compassion, drive excellence, and be part of a team that treats each other like family.
Key Responsibilities:
Provide daily clinical oversight of patient care services.
Coordinate referrals and admissions with internal and external sources.
Evaluate patient needs and agency capabilities before admission.
Supervise care planning, partner assignments, and care delivery.
Ensure compliance with regulatory standards and agency policies.
Mentor and coach clinical staff to promote morale and retention.
Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Qualifications
Required:
Active, unrestricted RN license in the state of practice.
Minimum of two years RN experience (preferred)
At least one year of Home Health experience (preferred)
Current CPR certification.
