Job description
Granite Peak Home Health in Great Falls, MT, is seeking a Clinical Manager (Registered Nurse) to oversee all home health patient care within the designated region, including assessments, staff supervision, and care coordination.
You will assign patients, manage referrals, maintain individualized care plans, lead weekly IDT and leadership meetings per CMS/PDGM guidelines, monitor quality and compliance, and support budgeting while developing staff and improving patient outcomes.
