nAs a Clinical Team Manager , you'll contribute to our success in the following ways:
nnnOversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
n
nnPlan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
n
nnEnsure timely start of care and ongoing compliance with applicable regulatory guidelines.
n
nnReview and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
n
nnConduct regular OASIS reviews to support accurate documentation and quality patient care.
n
nnProvide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
n
nnPartner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.
n
n nWhat is Required?
nnnAssociate degree in a Nursing related field, Bachelor preferred
n
nn2 years of home care experience in Home Health or Hospice environment
n
nn1 year of supervisory experience required
n
nnCurrent, unrestricted RN license valid for the state of work
n
nnAbility to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n
nnMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
