Job description
Carle Health seeks an RN Clinical Transitions Specialist to coordinate safe, efficient patient transitions across care settings. In this role, you’ll collaborate with physicians, nurses, case managers, and community providers to develop and implement discharge and transition plans, educate patients and families, and reduce readmissions. You’ll review clinical data, identify barriers to care, arrange follow-up services, and ensure compliance with quality and safety standards.
