RN Director of Case Management: Discharge & Care Coordination

Job description

LeoForce is seeking a Director of Case Management in Yuma, AZ to lead the hospital's case management department, oversee utilization review, discharge planning, and care coordination for a patient population with complexity.

The role requires an RN with a BSN, an active RN license, and at least 3 years in hospital-based case management; CCM/ACM is preferred. You will supervise an 11-person team and collaborate with physicians, nursing, social work, and leadership.
