Job description

PAM Health, LLC is seeking a Director of Case Management (RN) to lead the hospital-wide case management program in Shreveport, LA. You will oversee discharge planning, care coordination, and review of medical records to optimize clinical and financial outcomes.

Qualified candidates hold RN licensure, BLS, and prefer BSN/MSN with case management certification. Prior management experience in IRF/LTACH is desired, with a focus on quality care and regulatory compliance.
