Job description
Wellpath / Correct Care Solutions in California seeks an RN Discharge Planner to manage daily clinical care, utilization and care management for CalAIM-eligible individuals, ensuring safe transitions and coordination with managed care plans.
The role requires an active RN license in the state, 2 years of discharge coordination experience, and BSN/ CCM preferred. Previous correctional/detention experience is valued, with opportunities for growth in a mission-driven setting.
