Job Description
Registered Nurse Diabetes Educator
Education and Experience
Graduate of an accredited nursing program; Education Verification is required.
BSN preferred.
Certification and Licensure Licensed as a Registered Nurse (RN) in the states of Illinois and Wisconsin.
Approved CPR certification required within 90 days of hire.
Certification as a Diabetes Care and Education Specialist (CDCES) required.
Position Overview
The Registered Nurse Diabetes Educator is responsible for delivering compassionate, culturally sensitive, and holistic nursing care to patients in a clinical setting. This role will support the overall nursing mission, practice values, and strategic plan of the organization.
Responsibilities may include variable shifts, weekends, holidays, and participation in on-call hours. Additional duties may involve floating to various units based on identified needs and clinical competency.
Essential Duties and Responsibilities Utilize the nursing process and collaborate with interdisciplinary team members in caring for patients.
Administer prescribed medications and treatments in accordance with established nursing techniques, policies, and procedures.
Document patient care according to standards of care.
Develop, initiate, and execute a patient plan of care tailored to the individual needs of the patient, in collaboration with the physician and clinic policy.
Participate in departmental performance improvement activities and shared governance.
Provide clinical expertise and guidance to other units/departments in support of the patient's longitudinal plan of care.
Utilize critical thinking to ensure patient safety and enhance patient experience, appropriately escalating concerns via the chain of command.
Engage in the professional nursing continuum process.
Act as a resource to staff on patient education programs/materials and address patient education issues or concerns.
Serve as a patient advocate addressing clinical, psychosocial, and financial matters.
Shift Information
Monday to Friday, Day Shift Hours.
Minimum Years of Experience
A minimum of 2 years of experience is required.
Certifications/Requirements Basic Life Support (BLS) certification required.
