Job description
Humana is seeking a Field Care Manager Nurse 2 (Maternal-Child Health) to assess member needs and guide families toward appropriate resources for wellness. You will perform telephonic and in-person assessments, develop individualized care plans, and collaborate with PCPs and community services to ensure high-quality, cost-effective outcomes.
The role is primarily field-based with a remote home office when not conducting visits, requiring up to 50% travel within Northern Louisiana regions.
