Job description
Humana Inc. is seeking a RN, Field Care Manager 2 (Physical Health) to assess and guide members toward resources for optimal wellness. This remote/home-based role conducts field visits and telephonic assessments, coordinating care with providers and families.
You will monitor progress, adjust care plans as needs change, and document outcomes while collaborating with the Louisiana Medicaid Team to improve member health and satisfaction.
