Remote | Full Time or Part Time | Monday–Friday Core Schedule | No holidays

About the Role
As a Care Manager, you’ll provide telephonic education and coaching to patients managing chronic conditions such as diabetes, hypertension, and obesity. You’ll be part of a supportive program designed to improve patient outcomes and lower healthcare costs — and you’ll be trained in health coaching methods to help patients make positive lifestyle changes.
Key Responsibilities
Identify and engage patients who could benefit from coaching.
Conduct telephonic outreach to explain the program and encourage enrollment.
Educate and motivate patients toward healthier behaviors by using motivational interviewing and behavior change science.
Document interactions clearly and accurately in our system.
Triage and refer patients to appropriate services as needed.
What We’re Looking For
Education & Experience
Active, unrestricted RN license in the US.
Previous experience in an employee wellness or health promotion role preferred.
Spanish-speaking ability is a plus.
Skills & Abilities
Strong communication, listening, and motivational interviewing skills.
Ability to follow detailed procedures and work independently.
Effective decision-making and problem-solving abilities.
Proficiency with Microsoft Outlook and Windows-based software.
Strong multitasking and organizational skills.
