Job description
Alluma in Crookston, MN is seeking an RN Health Coach to provide whole-person care addressing medical and behavioral health conditions. You will evaluate medical history and integrate physical health with behavioral health plans.
You will screen clients, manage short-term medical needs, coordinate with providers, and guide transitions between inpatient and community care. A valid MN/ND RN license and CPR are required, with 2+ years of nursing experience preferred.
