Job description
WellMed, part of the Optum family, is seeking an RN Healthcare Coach to join our San Antonio team. This role focuses on assisting high-risk patients to navigate the healthcare system, educating and advocating for improved quality of life while collaborating with multidisciplinary teams.
You will perform nursing duties within RN scope, support transitions of care, and contribute to care plans with physicians, nurses, pharmacists and social workers.
