Job description
CarePartners, a Connecticut-based home healthcare provider, is seeking an RN to deliver skilled nursing care to patients in their homes. The RN will perform assessments, medication management, wound care, and chronic disease monitoring while developing individualized care plans. You’ll educate patients and families, coordinate with an interdisciplinary team, and document care in electronic records.
