Job description
Mount Sinai Health System is seeking a registered nurse to identify, evaluate, and enroll patients into Mount Sinai at Home programs. The role is on-site, integrated with clinical teams across hospitals, and involves coordinating admissions from the ED and inpatient floors.
You will communicate with patients and care teams to ensure home safety and eligibility. The ideal candidate is a NY-licensed RN with BSN/ADN and BCLS, with 1+ year of RN experience.
