Job description
A mission-driven healthcare organization is looking for a Registered Nurse (RN) Care Manager to provide exceptional patient-centered care across various settings, including clinics and patient homes. The RN will be responsible for assessing patient needs, coordinating care, and facilitating communication among providers. Candidates must have an active RN license and a BSN is preferred. Experience with complex populations and strong communication skills are essential. The role includes some travel to patient homes and may offer hybrid flexibility.
