Job description
FreedomCare is seeking a Registered Nurse (RN) Home Health Case Management for a nursing job in Wolcott, Connecticut. The Care Manager, RN conducts initial in-home assessments and coordinates care with patients, caregivers, and interdisciplinary teams to ensure safe, effective, person-centered services.
New graduates welcome. Requires a valid CT RN license and flexibility to travel within the assigned area. This staff role offers 40 hours per week, 8-hour day shifts, and ongoing employment.
