Job description
Your Health is seeking a Registered Nurse to serve patients across the Columbia area as a Nurse Case Manager. The role includes home and facility visits, coordinating with providers, and telemedicine initiation.
A valid RN license and reliable transportation are required as travel is daily, with work Monday–Friday 8–5. We serve South Carolina and Georgia with comprehensive primary care, specialty services, and pharmacy support.
