Job description
FreedomCare in Berlin, CT is seeking a Registered Nurse (RN) Home Health Case Manager for an ongoing 40-hour/week staff position. You will conduct initial in-home assessments, develop and revise plans of care, and coordinate with physicians and care teams. You’ll provide patient and caregiver education, monitor outcomes, and ensure regulatory compliance.
