Job description
Personal Touch Home Health Care Services Inc. is seeking an RN Home Health Case Manager to provide hands-on patient care at clients' homes while coordinating with healthcare providers and insurers. You will develop personalized care plans, supervise LPNs, and manage caseloads including start of care and evaluations.
