Job description
FreedomCare in Glastonbury, CT is seeking a Registered Nurse (RN) Home Health Case Manager for ongoing, full-time work. You will conduct initial in-home assessments, create and adjust plans of care, and coordinate with physicians and care teams to ensure safe, effective service delivery.
