Job description
FreedomCare is seeking a Registered Nurse (RN) Home Health Case Manager in Manchester, Connecticut. The role focuses on in-home assessments, care coordination, and ongoing management to ensure safe, person-centered care.
The RN will conduct initial assessments, activate services, and collaborate with interdisciplinary teams. Valid CT RN license and flexibility to travel are required. New graduates welcome.
