Job description
Phoenix Home Care & Hospice in Kirkwood, MO is seeking a Registered Nurse Home Health Case Manager. You will manage a caseload of home health patients, coordinate with therapy teams, and build personalized care plans aligned with physician orders.
This full-time, Monday–Friday role offers a base of $78,000–$88,000 with a $10,000 sign-on bonus, weekly pay, and comprehensive benefits including 401k, medical, dental, vision, life insurance, and PTO.
