Job description
Rocky Mountain Home Health and Hospice seeks an RN Case Manager to provide comprehensive patient care in the Murray, UT area. The role focuses on coordinating care, performing visits, and managing OASIS collection and lab reporting.
This position requires a current RN or LPN license, CPR certification, and strong communication skills. The RN Case Manager will interact with physicians and office staff, oversee visit frequency for episodes, and participate in case conferences.
