Job description
Sentara Home Care is seeking a qualified RN to join as a Home Health Coordinator in Woodbridge, VA. The role is full-time, day shift, with mileage reimbursement for patient visits.
The RN will coordinate and supervise nursing care, participate in interdisciplinary team meetings, and perform initial and periodic assessments to ensure quality patient care.
