Job description
Sentara Home Care in Newport News, VA is seeking a qualified RN to join as Home Health Coordinator. This full-time day shift role coordinates nursing care, conducts patient assessments, and collaborates with the interdisciplinary team to support health maintenance and disease prevention.
Active Virginia RN license and at least 1 year of nursing experience are required, with BLS due within 90 days. Mileage reimbursement is provided for patient visits.
